Walking through a neonatal intensive care unit feels calm on the surface. Machines hum quietly. Nurses move with precision. Tiny babies fight for strength. What you don’t see is the emotional weight carried by the mothers watching from behind the glass.

The Hidden Toll of the NICU

For mothers of premature or medically fragile babies, the NICU becomes an emotional battleground. Studies show that up to 70 percent experience anxiety or depression. Many show signs of PTSD long after their baby has gone home.

“Having a baby in NICU can be one of the most emotionally isolating experiences a mother faces,” says Sr Londe, independent midwife and Vital Baby South Africa advisor. “You’re told to be strong. But inside, you’re scared and feel powerless.”

Separation Makes Healing Harder

NICU-related mental health challenges often begin immediately. The shock of medical uncertainty, the struggle to bond, and strict hospital rules add layers of stress.

“You may only be allowed to touch your baby for minutes at a time,” says Sr Londe. “That separation can affect bonding and confidence.”

Warning signs include:

• Feeling numb
• Difficulty sleeping
• Reliving traumatic moments
• Feeling disconnected from your baby

Many mothers ignore these signs, believing they must cope alone.

Breaking the Silence Around Maternal Mental Health

“There’s still stigma,” says Sr Londe. “We need to encourage mothers to speak up and understand they’re not alone.”

Support can include:

• Talking to a healthcare provider
• Joining a NICU support group
• Seeing a perinatal mental health specialist
• Connecting with parents who have been through the NICU journey

Hospitals are adding peer-support programmes to help families navigate the emotional strain.

“It’s okay to need help,” she says. “You’re not failing. You’re processing an extraordinary experience.”
